The Foundation funds only 501(c)3 organizations located within the five boroughs of New York City.
Organizations may apply once a year. Only one category per organization.

No supplementary paper materials are accepted.
The foundation cannot accept applications from organizations with a fiscal sponsor or whose tax-exempt status is pending. Organizations applying for a grant must have received an IRS determination letter.

Step 1: Log in or Register
Enter Applicant and Organization Contact information including a current mailing address and telephone number.
All applications must include:
1. Executive Director’s name, mailing address, telephone and email
2. Organization’s mailing address, telephone and website
3. Name and title of person completing the application, telephone and email
For grant writers and consultants — please include the Executive Director’s information in the Organization’s Contact section.
If your organization has previously applied online — please call us if you have questions about your organization’s account or if your organization has recently moved. We need to have the current contact information for all applicants and organizations.
Step 2: Click Apply

The foundation has three deadlines per year. The online application is available one month prior to each deadline. See deadlines.
Step 4: Answer Questions and Upload PDF documents

The online application closes at 5pm on April 15, August 15 and December 15. Please note that if a deadline falls on a weekend, the deadline is extended to the following Monday at 5pm.
Please print or save (PDF) your application for your records. Upon application submission, applicants receive an automatic confirmation email.

Q1. Project/Program Name
Enter project or program name.If the organization is applying for General Operating Support, please enter GOS.
Q2. Grant Purpose
Choose the area that best represents the organization’s request.

Please select borough(s) where organization is located and where programs are served.
Q4. History with LAF
If organization has received funding in the past, please list funding year(s) and grant amount(s) starting from the most recent grant received.
Example: 6/14: $10k; 3/12: $5k; 2006: $2k
Q5. Organization Budget Amount
Indicate the dollar amount of the organization’s budget for the current fiscal year.
Q6. Program Budget Amount
If the organization is applying for Program Support or Other, indicate the dollar amount of the program budget for the current fiscal year.
Q7. Amount Requested
Provide the dollar amount requested. A request must be proportionate to the program and organization’s budget. The Foundation’s grants range from $5,000 – $25,000.
Q8. Fiscal Year/Operating Cycle
Please indicate organization’s fiscal year or operating cycle.
Calendar Year: 12 consecutive months beginning January to December.
Fiscal Year: 12 consecutive months beginning July to June.
Q9. Percentage Across Funding Sources
Please list percentages for current funding sources.
For Q10-Q11: It is recommended to use MS Word count (under tools) before copying and pasting the text to the Mission & History, and Proposal sections of the application.
Q10. Mission & History (4,000 characters maximum)
Provide a brief description of the organization’s mission and history.
Q11. Proposal (6,000 characters maximum)
Provide a brief proposal which outlines the project/program, for which the funds are sought.
This should include a detailed account of how the funds will be utilized, a history of the program, and its goals and objectives.
All applicants are required to submit a proposal including organizations applying for General Operating Support.
Q12: Links to Websites & Images (optional)
Provide links to websites and/or images in reference to your proposal.
For Q13-Q18: All files must be saved as PDF documents in the following order: organization’s initials_type of file.pdf (Example: ABC_board.pdf)
Q13. Board of Directors (length of service)
Upload a list of the organization’s Board of Directors as a PDF file (2MB file maximum).
The list should include each board member’s occupation/title, length of service and term limits if applicable.
Upload the organization’s 501(c)3 determination letter as a PDF file (2MB file maximum).

Q17. Audited Financial Statement
Upload the organization’s most recent audited financial statement or signed 990 as a PDF file (5MB file maximum).
The audit or signed 990 should not be older than 15 months.
Organizations with a budget over $750,000 must upload an independent auditor’s report not a 990.
Q18. Funding sources list by type (received & projected)
Upload a comparison list of all received and projected funding sources for the organization’s FISCAL year (current year, last year and PROJECTED next year) as one PDF file (3MB file maximum).
1. Organize list by type of support: foundations, corporations, government, individuals, board of directors, events.
2. Include foundations and corporations name and dollar amount. For individual and board support, only list the aggregate dollar amount.
3. Specify if amount was received or if it is projected.
4. If organization is applying for Program Support or Other, please include funding sources for the entire organization and for the specific project/program.
